If you'd like to improve your evaluation skills, read this excellent article, How to Give a Killer Evaluation on lifehack.org.
After reading the article, I created a simple evaluation form -- as specified in the article. It should simplify your life as an evaluator. It focuses on the key ingredients that every good evaluation has to have: positive comments, suggestions for improvements, and a positive summary.
